Challenge in My Life: How I Dealt with It

I Just decided to take some medicine and went to bed. The pains still continue to happen the next day. So I went to the emergency room. I told the doctor my situation. I had taken some x-rays. About thirty minutes later the results were back. The doctor had discovered a large mass In my stomach and wasn't for certain. So I was recommended to the Broody School of Medicine. That place Is thirty minutes from where I live. The doctors there looked at my x-rays and did some more tests on me. The BOB-GUN had found an ovarian tumor on my ovaries. Then I was explained to that I was going to have immediate surgery. I never had a surgery beforeI was scared for my life. The doctors also mention that my being overweight that puts a risk with the surgery. The first thing I thought with that being stated I knew I was going to die. As the days went by I had to go the doctor every two weeks. It was like if I was having a baby. Matter of fact I look like I was pregnant with twins. I guess that was no thing but all that the fluid from the tumor. I would cry everyday and I became depressed. I began to get weak each and everyday. So I had to get a blood transfusion. I remember staying In the hospital the entire weekend. I did not want to all because it was holiday. It was Labor Day weekend.I wanted to stay home and enjoy my family. Once I had received the blood in my body I felt much stronger. Then it was time for my surgery. I remember my surgery was September 7, 2005. I was nervous, shaky and scared. My family was by my side. I remember waking up the next day after surgery. When I woke up I thought it was still Wednesday the day I had surgery. Scary right! I had all types of tubes and things I was hooked up to. I was in the trauma center in the intensive care unit. I felt awesome. God is so good. There was no pain my body. Everyday I still think about what I went through with the sickness that was In my DOD.There was so much pressure and pain all on the Inside In my body. The onl y thing I can't do now Is to have kids. That still bother me but I am learning to overcome. I want to adopt children very soon. My plan Is to begin the adoption process when I am 30 years old. Has changed my life dramatically. Because if I didn't have faith in me and God on my side I might be lying in my grave or my body will still be with pain. Challenges happen for a reason. It can be for good or bad. People are put to a test to overcome challenges. Most of all we deal the challenges and overcome it with a success.

Shadows of Night: The Fear of a Child

In Annie Dillard’s book, An American Child; chapter two describes the fear she had as a child, of the night shadows that would appear on her walls. Dillard was five years old and shared a bedroom with her little sister Amy, who was two at the time. When Dillard describes her little sister sleeping, I can picture her clearly in my mind. Dillard writes; â€Å"even at two she composed herself attractively with her sheet folded tidily, under her outstretched arm, her head laid lightly on an unwrinkled pillow, her thick curls spread evenly. (21) Another wonderful example of her descriptive writing is when she is telling of the â€Å"thing† that she is so afraid of at night in her bedroom. Dillard states that, â€Å"the damned thing entered the room by flattening itself against the open door and sliding in. It was transparent, luminous oblong; I could see the door whiten at its touch. † (21) This â€Å"thing† that Dillard also refers to as â€Å"it† also had sounds of a â€Å"rising roar† (21) as it moved across her walls. Dillard finally figured out that her â€Å"thing was a passing car whose windshield reflected the corner streetlight outside. † (21) It wasn’t until she was outside one day, that she heard the familiar sound that she hears at night when the â€Å"thing† appears on her walls. Dillard’s writing shows how the human mind and imagination can exaggerate even the simplest things, until we can see in front of us what the true reality is.

High Renaissance Essay High Renaissance art in Florence, Italy did not last very long, but made an enormous impact on the art world. In the paintings of this era, artists put emphasis on personality and the mental state. Perspective, classical cultures, and anatomy were of a greater interest to the artists of this era. The analysis of Guido Renis Portrait of Cardinal Roberto Ubaldino, Papal Legate to Bologna, Leonardo Da Vincis Mona Lisa, Michaelangelos David, and Raphaels Baldassare Castiglione could offer insight into the paintings of the High Renaissance artists in Italy with respect to personality and the mental state, detail, and use of color. â€Å"High style† was the mainstay of this period of the Renaissance. High style art was characterized as the idealistic, perhaps heavenly. High style had a respect for ancient imperial standards of wealth, magnificence, and grandeur. (Emison, xxx) During the Early Renaissance, artists had to choose between realism and spirituality. During the Early Renaissance, figures were painted to look so real that many thought they lost their spirituality. From then on, an artist had to choose to either make his figures realistic or spiritual. When the High Renaissance began, Leonardo Da Vinci believed that an artist did not have to make that choice. For example, when Da Vinci painted the Last Supper, he made the figures in his painting realistic, however the figures had an undeniable spirituality about them. Da Vinci did not choose between realism and spirituality, he used both. (â€Å"High Renaissance†) The High Renaissance lasted from about 1490 to 1530. This era unfolded amidst a fury of mounting political and religious tension. This affected artists, as well as patrons of the arts, throughout Italy. Political tensions were high. In 1512, Copernicus determined that the sun, not the earth, was the center of the universe. Christopher Columbus discovered the Americas in 1492 and Magellan circumnavigated the world in 1522, which dismissed the idea of a flat earth. All of these discoveries changed many assumptions about human life and rocked the foundation of theology. (â€Å"Italian High Renaissance Period†) In those years, the masters created a new profession with its own rights of expression and its own character. Regional differences existed during this time, not only between Northern Europe and Italy, but within Italy, with the leading artistic centers being Rome and Florence. During the High Renaissance period in Italy, most artists worked for the papacy. These artists explored an interest in perspective, personality, and anatomy that became the main characteristics of the High Renaissance era. In Florence, artists emphasized preliminary drawings, based on careful and detailed preparation, before the painting began, whereas in other parts of Europe, artists just began painting. This is part of the reason why so many regard Italian Renaissance art as the finest art in the world. (Kleiner, 579-623) This artistic period represented a culmination. The High Renaissance artists took all of the artistic explorations of the Proto-Renaissance artists, which flourished during the Early Renaissance, and used these explorations to its full capability. (Esaak) Guido Renis Portrait of Cardinal Roberto Ubaldino, Papal Legate to Bologna was painted in 1627. Oil on canvas and 77 ? by 58 ? inches, it resides at the Los Angeles County Museum of Art. The subject was placed in front of a purple cloth, which insinuated honor, next to a velvet-covered table. The subject had obviously been writing, noting the letter and quill pen in the inkwell. The room opened to the left, revealing a courtyard. Rich colors, depth, and great detail add to the perception of the artist and the personality of the subject. The juxtaposition of the brightly-colored clothing with the dark background keeps the viewers eye on the subject. The subject looks thoughtful, looking at the viewer, giving the air of an intelligent man. Great detail was used by Reni. For example, the flecks of white in the subjects beard, the lines on his hands, the patterns and sheen of the clothing, and the fringe on the bottom of the chair shows that Reni wanted to capture every aspect of the subjects personality. It shows that the subject enjoyed the finer things in life, while looking perhaps within himself for the answers to lifes mysteries. Raphaels Baldassare Castiglione, painted around 1514, oil on canvas, with the dimensions of 2 6 ? † by 2 2 ? †, typifies High Renaissance portraits in the attention the artist paid to the subjects personality and mental state. The painting makes use of muted tones and Raphael favored clarity over obscurity. The muted tones fit the mood and temper of the subject. (Kleiner, 587, 623) The subjects facial features are clear, lending to the notion that Raphael focused on the subjects personality. The subject is dressed soberly, yet splendidly, and looks directly at the viewer. He has a thoughtful and kind gaze. This shows his humanity and lends an emotional openness to the painting. The subject looks sensitive and vulnerable, and his clothing suggest a certain softness to his character. This gives the viewer the feeling that the subject is equal to the viewer. The subject seems open enough to sit with and have an intellectual conversation. (Jones) Leonardo Da Vincis Mona Lisa, painted in the sixteenth century, is an oil painting on poplar panel. The painting depicts a woman with an ambiguous expression. This work has been at the center of study, scrutiny, parody, and myth for many years because of the fascination with the mysterious woman. The subjects expression seems mischievous, and she is looking directly at the viewer, a quality of High Renaissance art. The colors are dark, lending to the mysterious notion of the woman subject. The subject is seated on a marble chair, and behind her, a body of water with some rocks visible create the backdrop. The juxtaposition of the dark background and the skin of the subject tends to keep the viewers eye on the subject. Michelangelos David statue, carved from a single piece of marble and finished in 1504, is the representation of physical male perfection. The Cathedral of Florence commissioned Michelangelo to sculpt the statue from a block of marble that was originally from another project. David was to originally be put up on top of one of the buttresses of the Cathedral, however, when the sculpture was finished, they decided to put it in front of the main government building in Florence, Italy. (â€Å"Michelangelo, David, 1501-4†) All of these artworks are portraits. An great artist who paints portraits must have the skill necessary to depict something beyond its physical appearance to convey the subjects life and character. All of these artists imbued their works with psychological insight and authenticity that other artists of the day could not match. This is what made these particular artworks part of the High Renaissance era of Italy. (â€Å"Captured Emotions†, 3) Figures in portraits that were painted during the Early Renaissance were in profile and cut off at the bust. This was not so with portraits of the High Renaissance. These portraits, such as the Mona Lisa and Baldassare Castiglione, were not painted in profile, but with the face frontal, and the shoulders ? , with hands included. This was done instead of the profile because when the face is frontal, the viewer gets a better sense of the subjects personality. This revealing of the personality in paintings was a main interest during the High Renaissance and in artwork since. (â€Å"Leonardos Mona Lisa†) As was explained through these four examples, High Renaissance art in Florence, Italy focused on subjects personality and minds. The artists wanted to capture what the subject was really like and how they thought. The point of the High Renaissance was to delve deeper, to bring out qualities, not only in the subject, but also in technique and color use, that made the work stand out and say something. High Renaissance artwork has influenced the world of art ever since. For example, when children sit to have their school photo taken, they sit the way the subject in the Mona Lisa did. These works have withstood the test of time and continue to provide insight into not only the subjects personalities and minds, but also the artists.

Follicular Lymphoma Case Study The clinical condition given was a 33 year old patient diagnosed with chemotherapy resistant advance follicular lymphoma. Lymphoma is a type of cancer of the lymphocytes, which belongs to the white blood cell family of the body.1 White blood cells circulate the body through an extensive network of lymphatic systems, illustrated in figure 1, which includes the bone marrows, spleen, thymus and the lymph nodes. Lymphoma is categorized into two, the Hodgkin lymphoma and the non-Hodgkin lymphoma (NHL). Follicular lymphoma belongs to the group of NHL that affects the B-cell lymphocytes, which plays a critical role in immune response by producing antibodies in human body. Under the view of a microscope, follicular lymphoma produces a nodule or follicular structure, as the name suggested. Microscopic image of follicular lymphoma can be seen in figure 2. Diagnosis Diagnosis requires a biopsy of a lymph node, involving the process of obtaining a small amount of tissue for testing in the laboratory.5, 6 However, biopsy alone will not be sufficient to diagnose the lymphoma. Other scans on the abdominal, chest, or pelvic areas by using techniques such as Computer Tomography Scan (CT-Scan), Positron Emission Tomography (PET-Scan), will aid diagnosis as well.7, 8 According to the National Comprehensive Cancer Network (NNCN) Guidelines, laboratory test which is inclusive of full blood count, uric acid level, serum calcium level, as well as liver function test are needed in order to confirm diagnosis.9 Follicular Lymphoma Stages and Classifications Staging is important in patient with follicular lymphoma as it divides patient into group and determines what kind of treatment is needed, or whether treatment is required. Generally, NHLs are divided into four stages, summarized in table 1 below. Follicular lymphomas categorized in stage II, III, and IV are termed as advanced follicular lymphoma, as the case of the patient in the scenario. 5, 6 Table 1: Stages of NHL in terms of regions involved. Stages Descriptions Stage I Only one of the lymph node is involved Stage II Two or more lymph nodes from the same side of diaphragm are involved Stage III Lymph nodes at both sides of the diaphragm are involved Stage IV Involves a number of lymph nodes, tissues or organs Furthermore, lymphomas can generally be described as Low Grade, Intermediate Grade, or High Grade, depending on the speed they grow, based on the Revised European-American Lymphoma (REAL) grading system1, 6. Generally, low grade lymphomas are slow growing, while high grade lymphomas are characterized by rapid growing tumor cell which requires aggressive treatment compared to low grade and intermediate grade lymphomas. Ironically, many High Grade lymphoma can be cured, while low grades lymphoma tends to reappear and contribute to relapses.6 Apart from the general lymphomas grading, follicular lymphomas have their own grading system, which are separated into three grades. According to the World Health Organization (WHO), the follicular lymphomas are categorized according to the number of centroblast (large follicular cell) present while the tumor was scanned by using the highest power of magnification on the microscope.1, 5, 6 The grades were summarized in table 2. Causes A research done by Bosga-Bouwer et al. over 30 patients indicates that genetic factors are the primary causes of follicular lymphomas. The research used southern blotting technique, polymerase chain reaction, and fluorescence in situ hybridization to confirm a translocation in the chromosome 14 and 18, which affects the bcl-2 proto-oncogene, and the main function of bcl-2 genes is to prevent apoptosis of B-cell lymphocytes.10 Overexpression of the bcl-2 gene results in prolonged survival rate of the cell due to the prevention of programmed cell death.1, 10 Signs and Symptoms Symptoms of follicular lymphoma include adenopathy, night sweats, weight loss, and fever. Adenopathy represents painless swelling in one the lymph nodes. Another important indicator includes elevated levels of Lactate Dehydrogenase (LDH), an enzyme that facilitates the conversion of pyruvate to lactate. 11 However, it is important to note that some of the patient lymphoma will be asymptomatic. Complications Follicular lymphoma will further complicate by increasing tumor bulk and threaten end-organ function. After a given period of time, it might transform into a more rapid growing lymphoma- the diffuse large B-cell lymphoma (DLBCL). In contrast, DLBCL is a much more aggressive form of NHL. According to a research done by Horning and Rosenberg, 25% to 60% of the patient will transform from follicular lymphoma into DLBCL12. A paper done by Lossos et al. suggested that the transformation to DLBCL includes multiple genetic mechanisms no single gene is responsible for the transformation13. Apart from that, another paper by Montoto from Cancer Research UK Medical Oncology Unit, demonstrates that the risk will be significantly higher (P-value = 0.02) if a patient was in an advanced stage of follicular lymphoma, which is the situation present in the case scenario14. It was also found that there are no statistically significant differences between the time of initiation of therapy and chances o f transforming from follicular lymphoma into DLBCL. If a patient with DLBCL is not treated accordingly, it is often fatal 5, 14. Furthermore, tumor of follicular lymphoma might relapse and further develop into chemotherapy resistant follicular lymphoma. Chemotherapy resistant lymphoma occurs when the lymphoma that has been responding to chemotherapy had begun to grow, resisting the effects of the therapy. The resistances are acquired through a number of mechanisms, namely genetic transformation of the B-cells. When this occurs, as seen in the case, the treatment option will be narrowed down, since chemotherapy is no longer an option.15 Incidence, Morbidity, and Mortality Rate Follicular lymphomas have an incidence rate of 2500 cases each year in the UK.16 It exhibits the second highest incidence rate among all the lymphomas, with the first being DLBCL. 17 It dominates 30% of all the NHL and it is the most common type of Low Grade NHL.1, 17 Prognosis Index is used to predict the treatment options and outcome of follicular lymphoma. A Follicular Lymphoma Prognosis Index, developed by Solal-Celigny, is currently being used internationally18. This prognosis index takes into account of 5 factors, which are age (>60 years), stages of follicular lymphoma (stage III IV), hemoglobin levels (low), numbers of lymph nodes/organs affected (>4), and levels of LDH (high) 17, 18. 91% of patient which suffers none of the abovementioned factors will live longer than 5 years. If the patient suffers from two or three of the factors in Follicular Lymphoma Prognosis Index, the rate of living longer than 5 years will drop to 78% and 43% respectively 18. In the UK, the median age of onset of follicular lymphoma was 60 years old, as compared to 67years old in the United States (US) 16, 19. In the US, according to the National Cancer Institute, rate of mortality was the highest among those from 75-84 years old, with a death rate of 33.7%. However, in those patient aged from 20-34 years old (case scenario patient aged 33years) mortality rate was only 1.6%. The average death rate of patient with NHL was 7.1 per 100,000 men and woman per year in the US19. Evidence Based Treatment Options of Follicular Lymphoma Follicular lymphoma can be considered incurable, but fortunately the growing tumors are sensitive to radiation and chemotherapy. Treatment options of lymphomas are highly based on severity of the symptoms and stages of lymphoma.5,20 If patient is asymptomatic, prescribers often employs a wait and watch approach, and according to studies, there is no difference in the survival rate between patients who were treated earlier and patients who were treated according to wait and watch method. 20 Treatments options are summarized in table 3. It was preferably use in Stage I or Stage II of the lymphomas which are localized.20, 21 The mechanism of radiotherapy includes usage of ionizing radiation such as beta and gamma rays to destroy the tumor cells and impairs their growth, while the body removes them through a natural process. Currently, the method employed in treatment of lymphoma tumor is known as the external beam therapy. 20, 21, 22 External beam therapy is done from a machine outside the patient, which is known and the linear accelerator. Radiation to body parts above the diaphragm is known as Mantle Field radiation, while radiation to parts below the diaphragm is known as Inverted Y-field radiation.22 A study by Aviles has shown that survival rate of a patient treated with radiotherapy for a 5 years period was 48%, and if used together with chemotherapy, the patients survival rate will be increased to 83%.23, 24 According to another research by MacManus, 40% of the radiotherapy patient remained disease free after 10 years. Chemotherapy Chemotherapy involves the usage of drugs to stop or slow down the growth of lymphoma B-cells, hence managing the lymphomas symptoms.5, 6 Chemotherapy drugs only target specific rapid-growing cells such as tumor cells, and impairs their ability to multiply and growth. 20 However, many cell lines in patients such as bone marrow, hair follicles, and the lining of the gastrointestinal tract are rapid-growing cells and therefore, it will be targeted by chemotherapy drugs as well, causing the side effects. Initial responses towards chemotherapy are usually high, and therefore, it is considered as the first line-treatment in treating NHL.20 Many protocols of chemotherapy combination were proposed, with the common ones being the CHOP and CVP. CHOP therapy includes the usage of four drugs- Cyclophosphamide, Doxorubicin, Vincristine, and Prednisone while CVP therapy uses Cyclophospharmide, Vincristine, and Prednisone.5, 6, 20 A research by Fisher had concluded that CHOP should be used as first line treatment for advanced grade non-Hodgkin lymphoma, with a result of 44% survival rate in 1138 patient after a period of 3 years.26 However, a more recent study by Hiddemann suggested that the use of Rituximab, a monoclonal antibody, together with CHOP greatly improves the response rate of patient to 96% (P-value of 0.011), and reduce the chances of treatment failure by 60%. The study, which was done on 2005, proves that R-CHOP is the superior frontline treatment of choice for advanced follicular lymphoma compared to CHOP. According to the British National Formulary 57 (BNF 57), the usage of CHOP will produce side effects such as oral mucositis, hyperuricaemia and tumour lysis syndrome. Nausea and vomiting is also the common side effects of chemotherapy drugs, and it may lead to refusal of further treatment. Bone marrow suppression and alopecia are also of the core problems of chemotherapy drugs. Monoclonal Antibody Therapy Monoclonal antibody therapy works by using antibodies that sticks to the surface of specific groups of cells, such as cancer cells, and trigger the bodys natural immune system to destroy these cells. 5 The advantages of monoclonal antibody compared to chemotherapy is that they are more specific compared to chemotherapy. Rituximab is used to treat follicular lymphoma, due to the mechanism of action of binding to CD20 protein- the abundant protein on the surface of B-cell. Rituximab causes the death of the cell by the induction of apoptosis. 29 According to the BNF 57 and North London Cancer Guideline, Rituximab is used to treat stage III and IV follicular lymphoma, or in those patient that had a relapse after treatment of chemotherapy, or those that other treatment options are exhausted.16, 28 Nice guidelines recommends that Rituximab should be given as a third-line treatment, and should only be given as first line with combination of chemotherapy to patient with stage III and IV follicular lymphoma. A meta-analysis research by Vidal et al. reveals that when Rituximab was given as a maintenance therapy, it significantly improves overall survival rate and the relapse rate of patient, compared to patient that did not have Rituximab as maintenance therapy.29, 30 Another research by Hauprock and Hess indicates that Rituximab improves survival rate if given as relapse therapy and maintenance therapy.31 When given together with CHOP chemotherapy, it greatly improves the overall outcome of a patient. Adverse side effects of Rituximab appears to be infusion related side-effects, namely the cytokine release syndrome, which is characterized by fever, chills, nausea and vomiting, as well as tumor pain. Radioimmunotherapy Radioimmunotherapy consist of one radioactive isotope and a monoclonal antibody to assist specific cell targeting. The radioactive isotope which is attached to the antibody will release radiation once the antibody is bound to the specific cell type, hence killing the cells.21, 32 This method is considered to be a more advance therapy compared to chemotherapy and monoclonal antibody therapy. There are two drugs available now in radioimmunotherapy, namely Bexxar and Zevalin.32, 33 Zevalin consist of two radioactive isotopes called indium-111 and yttrium-90. The purpose of indium-111 in the drug is to enables physician to view the paths of the drug in the body, while yttrium-90 is the therapeutic portion of the drug. Zevalin is administered via the intravenous route together with Rituximab over a time of ten minutes, over a course of a week. The main side effect of Zevalin is lowered blood cell count, normally observed 4-6 weeks after the treatment. However, Zevalin is considered to be advantageous in terms of side-effects as it does not exhibits the normal side-effects of chemotherapy drugs, such as alopecia, nausea and vomiting. In a clinical trial involving patients with NHL, Zevalin manage to produce a response rate of 70-80%. Furthermore, in the same trial, Zevalin produces responses in patient who no longer respond to chemotherapy and Rituximab.32 A review by WitZig showed a response rate of 82% to Zevalin in patient with low grade NHL. The advantages of Zevalin Therapy are that it only uses a single dose of radiation and it is being well tolerated by the patient, which was proven in the study.34 In another randomized controlled trial done by WitZig and Gordon et al., Zevalin was proved to be more superior than Rituximab, with an overall response rate of 80% compared to 55% in treating follicular lymphoma. Apart from that, another immunotherapy drug called Bexxar, is a combination of Tositumomab and Iodine I 131 Tositumomab. Tositumomab is a monoclonal antibody targeting the CD20 protein in the B-cell Lymphoma, while Iodine I 131 Tositumomab is a radiolabeled derivative of the monoclonal antibody. Beta-radiation released by Iodine 131 is responsible for killing of the tumor cells. Bexxar is given in two different sets of intravenous infusion, two weeks apart.   Common side-effects of Bexxar include suppression of bone marrow, which is characterized by low blood counts. Bexxar might also cause hyperthyroidism and anti-murine antibody formations.   A research done by Kaminski et al. shows that Bexxar produces a very high response rate of 95% in 76 patients who enrolled in the study. 75% of the patient in the study still had a complete remission even after 5 years. This research of Bexxar rivals any kind of therapy previously used in treating follicular lymphoma, including chemotherapy, as Bexxar therapy will be completed in just one week, and proves to be more effective. Another study, by Jacene, comparing Bexxar and Zevalin, reveals that Bexxar will cause less bone marrow suppression compared to Zevalin, although both were generally well tolerated.   Treatment Recommendation for Patient in Case Scenario According to the patient in the case scenario given, he is currently 33 years-old and he developed chemotherapy resistant advanced follicular lymphoma recently. Advanced follicular lymphoma indicates that his condition is in either stage III or IV, involving lymph node on both sides of the diaphragm and/or organs. In this case, since the follicular lymphoma developed resistance, chemotherapy is no longer an option. According to the BNF and the NICE guideline, Rituximab monotherapy could be given to patients who have relapsed stage III or IV follicular lymphoma, which developed resistance to chemotherapy. 28, 29 It is recommended that the patient takes 375mg/m2 of Rituximab over a period of 21 days, according to the instructions in NICE guideline. The patient has to be on it for 8 cycles which is approximately 6 months.   Rituximab was proved to induce better response and improve overall survival rate in follicular lymphoma patient according to the meta-analysis of randomized trial performed by Vidal et al. and another study done by Hauptrock and Hess (mentioned above in treatment). Another study by Monila further supports the usage of Rituximab as it increase both response rate and survival rate as well as improving the long-term prognosis of follicular lymphoma patient. All these evidence validates the treatment option of using Rituximab in the patient. The cost of Rituximab based on the evaluation of Assessment Group Model in the NICE guideline is approximately  £8500 per life year gained in patient younger than 60 years-old and  £9700 per life year gained in patient aging 60 years-old and above. It is considered cost effective based on the evaluation of the NICE guideline. If treatment using Rituximab proves to be unsuccessful, radioimmunotherapy drug Bexxar will be recommended. Although Bexxar is still considered a new drug, the clinical trials done by Kaminski shows a remission rate of 75%. Bexxar had also been evaluated in one of the study, which shows that 86% of the patient achieved a complete response in Stage III and IV follicular lymphoma. Conlusion In conclusion, patient with chemotherapy resistant advanced follicular lymphoma should be treated with Rituximab, followed by Bexxar and Zevalin. All in all, with the new advances in radioimmunotherapy, the joint effort of health care professionals and the utilizing of guidelines with evidence-based research, patient with the follicular lymphoma, although incurable, will still be able to lead a healthy and fruitful life.

Seeking Solace in The Bean Trees Many aspects of life are explored in Barbara Kingsolver's novel, The Bean Trees. A young woman named Marietta Greer from Kentucky wanted to strike out on her own, leaving behind everything she ever knew, just to start a new life. Many children want to do this at an early age so they can experience life on their own yet they don't realize the dangers involved.. Everyone that leaves the solace of their own home needs loving support to keep them going through life. Marietta Greer starts her journey by heading west across the United States in a run-down old car. She decides at the start that wherever the car breaks down will become her new home. When she stops at a diner one night for something to eat, she is suddenly left with a baby dropped off by an elderly Indian woman with no reasons why. Marietta tells her "If I wanted a baby I would have stayed in Kentucky" (24). Continuing her lonely trek, she changes her name to Taylor and ends up keeping the child and caring for her. When she finally arrives at her unknown destination, Taylor gets a job at a tire company working for a nice lady named Mattie and moves in with Lou Ann, also a single mother. She develops close friendships with them, and they all help each other through good times and bad. Taylor starts getting stronger feelings for the child, Turtle, and eventually adopts her through deceiving means with her new friends Estevan and Esperanza. Taylor keeps in touch with her mother, and she was thrilled her mother was supportive of her decision to adopt Turtle. There are some things I would have changed about the novel to make it more interesting reading and to broaden the type of audience it addresses. In my opinion, the book was kind of targeted at a feminine audience, especially single mothers. When Taylor and Lou Ann get together, Lou Ann sounds kind of pathetic saying " Who in the world would want to move in here with us" (102), yet they develop a strong friendship regardless. I had kind of a hard time focusing on what was going on and who was doing what mainly because the story wasn't interesting enough to attract my attention. Perhaps by throwing in a little more suspense or more action, the novel would have been more fun to read.

Feminism in Antigone and A Doll’s House Essay -- Literary Analysis, So

Eleanor Roosevelt once said, â€Å"A woman is like a tea bag. It's only when she's in hot water that you realize how strong she is.† This is very evident in both Antigone and a Doll’s House. Antigone is a Greek play by Sophocles about a young girl who decides to take on the task of burying her traitorous brother to honor the gods even though her tyrannical uncle Creon has outlawed the burial. A Doll’s House by Ibsen follows Nora Helmer, a housewife who has borrowed money without her husband’s knowledge or consent for the purpose of taking him to Italy for medical reasons. It outlines the internal and external conflicts she experiences in the final days of her secret. It is important to understand the roles of women in both plays because during the time periods they were written, women were held to very different standards and were far more oppressed than men. There are many examples of nonconformity of women in Antigone and A Doll’s House in the ways that the main characters rebel against what is expected of women. Nora and Antigone are headstrong, sharp-witted, and willing to break the law for love, three character traits that were uncommon and almost frowned upon by society for women in history. Nora and Antigone were both very headstrong women. During the time periods of both plays, women were expected to be subservient and share the same opinions as their husbands. The two lead women in the plays show their obstinacy and ability to take care of themselves in many instances. One example in Antigone’s story is when she says (to her â€Å"stereotypical woman† sister) of her plan to bury Polynices, â€Å"He is my brother and—deny it as you will—your brother too. No one will ever convict me as a traitor, (Sophocles 18). She then goes on to... ...use she knows that she will be following the gods’ law and honoring her own flesh and blood by burying Polynices. All in all, the women in the plays go against the rules and practices of society so that they can do something for the ones that are close to them. On the whole, Nora Helmer and Antigone certainly prove that women have the ability to do anything a man can do, just as well. The roles of women in Antigone and A Doll’s House are certainly different than the roles of men, and the two main women in both plays transcend the expectations and beliefs of what women can and cannot do. They prove this in several ways, including the fact that they were determined, very intelligent and clever, and wiling to break the laws of man for loved ones. Antigone and A Doll’s House are great works of classic literature that portray women as strong and intelligent beings.

The Spanish influenced countries of Latin America and South America are seeing the largest increase. The increased interest in the Hispanic culture by society is driving many consumers to want to escape to the sandy beaches and the tropical oasis. The other trend is the decrease in the African and the Middle Eastern Pipelines. Much of this can be contributed to the political unrest in these countries that is covered by the media. While many still want to go on vacations to their holy lands for religious purposes or visit many ancient areas of art and documented history not many consumers are wanted to travel with young children to these areas due to the unrest and military presence and rather are looking for rest and relaxation rather than additional intellect and experience. b) Based on the headlines and the articles on Hotel Online I have found a trend to be the renovations of hotels that have been long standing. The renovations of these hotels are focused on the consumer and the upgrades to the lifestyle changes and health and environmental awareness across the world. Many hotels are going green and adding markets or dining options that are also green and health conscious. This is important as the world begins to become more aware environmentally of what the waste is doing to the planet that we want to have around for our children many more people are recycling and finding innovative ways to reuse and cut back. The Ritz Carlton website focuses on the room amenities and what you will experience inside the room for the duration of the stay and highlights that in scrolling photos. Rosewood highlights a full screen picture that is very different from most websites and the actual background of the site is the photo that changes with different cultural experiences that you could have as a guest at the resort in that country. b) In my opinion the hotel with the best marketing strategy is Rosewood Hotels & Resorts. This is because as the website features the different locations you see photos of actual people interacting with another from that particular region or culture. You see â€Å"yourself† in the eyes of the people who are living the experiences captured. This is very effective as it is assumed the room with be gorgeous and the amenities top notch so why not showcase the experience the guest can choose to have. c) These hotels cater to the wealthy and the people who do not look at the price of the stay but rather the experience the stay will give them and the social status of the location as well. These hotels are large and expensive as well as the contemporary dà ©cor and the sought after privacy that the guest will get. The Choice and Cendant hotel groups market their price on price and breakfast as well as comfort ability and internet amenities as they are serving the population of business personnel and families who will most likely spend most of their time out of the room at local attractions rather than entertaining in their own suite as is possible with the luxurious facilities.

Music Essay African music roots start in every aspect of society. Their culture is formed from there environment hunting but mostly on their tradition. Their music reflects this not only is it entertaining it serves as a way to pass knowledge and stories to the next generation. African music has a variety of different instruments that they use throughout their songs. Music in African culture also has very strong spiritual and religious meanings. The recent history of African musical influence has played a large part in encouraging the current trend toward African musical appreciation. During the 1950s, when the Africans experienced rapid urbanization and a relatively booming economy, the two French-speaking colonies of the Congo area witnessed the birth of an African version of the Cuban rumba played by small American-style orchestras. Some artists that are African are Afro-National, Khaled Agag, Eric Agyeman, and Mahmoud Ahmed. Other than using their voices, which has been developed to use various techniques, they use a wide array of musical instruments are used. African musical instruments include a wide range of drums, slit gongs, rattles, double bells, and melodic instruments including string instruments. Other percussion instruments include many rattles and shakers, such as the kosika, rain stick, bells and wood sticks. Also, Africa has lots of other types of drums, and lots of flutes, and lots of stringed and wind instruments. The traditional tribal African music has deep meaning in its lyrics along with its rhythm and beat, its lyrics often tell of struggles, religion, and day to day life. The Africans would perform rituals to try to please the gods, these rituals would include music made by the high priest the instruments used would be tribal drums and a rain stick. The lyrics would be similar to Christian hymns in our modern culture

Dear Mayor, Being the Chief of Police for Los Angeles, I am strongly concerned at improving all the records, and the manner of using and accessing them.   With this goal, I propose computerization of all the police records as the new system.   Efforts towards computerization of all the police records must start early at this point, considering the fact that other data have been computerized already. In the information age, advanced technology can be taken advantage of without encountering significant risks.   Many businesses have already relied on computerization, such as banks, stores, libraries, and other companies that have complex business processes.   This fact has proven the advantages of computerizing all the records. Maintaining law and order is a difficult and has different aspects.   Despite that, several tasks involved in police offices are still routinary or can still be optimized further.   These tasks, being performed repetitively, are prone to human error, more so when workers are already exposed to long working hours and under psychological fatigue.   On the other hand, unintentional mistakes could have been avoided with the help of automation. In addition, instead of investing on personnel just to maintain records, human efforts could be focused on more important aspects in the government.   Instead of assigning people to perform tasks that a computer can do faster and more accurately, computers can be purchased in place of workers that must be paid every month. Details on fugitives, arrest records, warrants, missing persons, stolen property, and alerts relative to terrorism threats must be easily accessible to the police, so the fastest manner of updating, storing, and accessing data is preferred.   To answer this concern, a database is an appropriate option because it is where data can be stored and then be arranged easily and conveniently. The view of the data may be organized; irrelevant details may be hidden from the view of a certain user who is performing a certain task only, so that searching is more efficient and more secure.   Databases are used in many applications, spanning almost the entire range of computer software.   They are the preferred method of storage for large applications with many users. Databases can be managed more easily than analog data (such as card catalogs).   Moreover, since the data that are usually recorded on paper would be converted to electronic data, data loss and dilapidation is not a significant problem anymore.   In digital format, copies may be made without sacrificing quality, unlike copying analog data (e.g., using photocopiers).   Data can also be maintained more easily in digital format. In addition to speed, accuracy is also a good asset of having computerized records.   If the data are standardized and more centralized, redundancy is avoided.   Since transparency is increased through the computerization of police records, confusion can be avoided. Developing specialized software may be expensive, but the same computer program is used to all the police stations, and it may be used for a long time.   If some aspects of the electronic election process must be enhanced in the future, the existing system can be used as a foundation instead of starting everything from scratch.   Therefore, the cost of labor (in terms of time, energy, and money) is reduced in the long run. Before implementing the electronic system, studies must be thoroughly conducted regarding the effectiveness of the system in real scenarios, risks involved in implementation, and the cost and benefits.   Interested university students who have the relevant background may be encouraged to participate in conducting the research. Like any method of keeping records, computerized records are still susceptible to fraud.   However, considering the fact that the processes are standardized and centralized, errors and fraud attempts are detected more easily. Computer networks may be used as a medium, but the system must protect itself against identity theft and vandalism.   To minimize the potential negative impact of this new system, standards must be established, and more laws against computer crimes and breach of security must be strengthened.   The private information can be encrypted before being sent, and decrypted after being received at the other end.   Both the encryption and decryption algorithms can be performed conveniently using the software. Because of this move to computerization, one of the basic goals must be to ensure that every division/section in the police department is equipped with computers.   However, security requirements must still be tightened, and the access levels of the police must depend on his position and his scope of responsibilities.   These measures would avoid unauthorized access to sensitive data. Only the data that must be seen by the public.   The system must also allow citizens â€Å"to easily access and obtain several types of documentation, such as information on tainted vehicles, information on tainted firearms, domestic servant verification, and instant passport verification.† (â€Å"National Crime Records Bureau†, 2007). This method of storing data into digital format is not less secure than analog data. Both methods are susceptible to breach privacy, and true data security can only be achieved if everyone involved in the process have maintained their integrity as a person as they develop solutions to update and maintain the records, and the solution implemented is complex enough to be deciphered by hackers. This proposal is a good starting point to let the police and the people realize the benefits of automated systems.   Aside from the aforementioned benefits of computerization of police records, other benefits that may seem less obvious at this point will certainly be useful. Harnessing and exchange of information on criminal operating in neighboring states would be more plausible as soon as other states have also decided to strive for computerization of police records.   In the future, having a reliable police record may even provide room for other important transactions such as electronic voting.   Hence, continuing studies must still be conducted to further improve the service of the government to the people. We must start the process of standardizing the way the criminal records are maintained across the state, in order to allow efficient sharing of information and facilitating greater coordination to the higher level. Thus, the change proposed is that it must be ensured that the individual gets optimal exercise depended on the severity of the problem, age, and gender. For instance, a study found that brisk walking more than ten blocks away in a day reduced the risk of cardiovascular disease in women. It is also suggested that different forms of exercise should be adopted for various people. The survey will seek to establish the kinds of exercises that the patients do regularly and how long they engage in the activities. Furthermore, the survey will seek to establish how they feel after the exercises and the notable health benefits that they gain thereafter. The respondents will comprise patients suffe ring from coronary heart disease, individuals of either gender randomly drawn from the hospital’s population. In addition to conducting a survey, interviews will be conducted with respondents being health experts directly dealing with the patients.
